 Memory Support Continuum Director   Manage the operations of the Cognitive Rehab unit and coordinates a multidisciplinary team. Directs innovative programs providing a holistic approach to resident care across the entire dementia continuum. Essential Functions & Responsibilities  Manages the operations of the Cognitive Rehab unit providing specialized care for Alzheimer’s disease and related disorders.  Works as part of Nurse Administration, providing backup for Healthcare Services Director, as needed and required.  Directs the development and implementation of innovative programs for residents experiencing dementiarelated disorders. Ensures programs are designed to focus on individual needs and promote physical, emotional, psychosocial, and spiritual wellbeing.  Directs and monitors the assessment process.  Provides leadership, direction, and development to staff responsible for all memory loss units (Crossroads, Pathways, MRE) including our Adult Day Care program (Cherished Place). o Works with department manager in areas such as nursing, activities, housekeeping, and food services in the selection, training, and evaluation of staff. o Provides feedback and direction to staff to promote quality services. o Promotes team building and staff participation in day-to-day operations.  Develops and facilitates ongoing educational programs and in-services on dementia related diseases for staff, families and the community.  Facilitates 12-hour dementia training including development and/or updating of materials, as needed.  Ensures appropriate documentation and care planning for all area residents in order to maximize resident care. Ensures an attractive homelike environment, which is safe and sanitary, is provided for residents, staff and volunteers. Includes: o Tours and monitors all areas. o Seeks to identify and prevent safety hazards, accidents and injuries.  Develops, monitors and responsible to achieve desired budget results.  Collaborate with the Health Care Services Director to ensure all units are appropriately staffed and services are in line with State and Federal Regulations, level of care, and census.  Maintain continuity of care by making rounds and providing information and guidance to the nursing unit, receiving reports on the status of all residents and services, visiting residents, assisting nurses with new and unusual treatments, deciding whether or not to call physicians, coordinating services with other shifts, admitting and transferring patients, ensuring appropriate communication and follow-up to residents, family and visitors and taking other action deemed necessary at the time.  Complete Lifesteps Assessment  Ensure nursing services are provided in a manner consistent with good financial stewardship.  Work with Health Care Services Director to develop nursing budgets.  Monitor nursing expenses, identifies trends and ensures operations are within budgeting guidelines.  Review census information.  Ensure resident RAI (MDS/Raps and Care Plan) process is competed for each resident and that Care Plan conferences are carried out with resident and family.  Promotes harmonious relationships with agencies, services, and persons with whom they regularly works, such as medical examiners, police, funeral directors, labs, pharmacies, x ray, and home health by careful scheduling personal contacts.  Maintains confidentiality in all matters.  Monitors quality service by enforcing proper procedures compliance in conjunction with legal requirements followed by assessing and evaluating for quality assurance.  Leadership role for quality improvement process.  Performs audits for Continuous Quality Improvement Committee.  Oversees notifications of IDPH within 24 hours when incident warrants it.  Assists in managing the reporting and investigations of abuse/neglect as requested by Abuse coordinator.  Assists with survey process.  Provides technical and clinical expertise to health care services including: In depth knowledge of regulatory guidelines, procedures, and eligibility factors.  Proceed
